*This is an open Career Service Health Educator position working fulltime at the Department of Health in Lee County.*

This is a highly professional position with lead responsibilities for linking Human Immunodeficiency Virus (HIV) positive clients to care in Lee County. This position will work with the current Human Immunodeficiency Virus/Acquired Immune Deficiency Syndromes (HIV/AIDS) and Sexually Transmitted Disease (STD) intervention staff to identify clients who are not in care. The position will locate the client and offer services including care and treatment. This position will work with community partners in the identification of those out-of-care clients and those clients who have been released from the county jail. This position is classified as a sensitive position. The incumbent handles protected health information on a need to know basis following Department of Health, HIPAA, and Information Security policies, protocols, and procedures. Employee abides by State/Federal laws governing health information. Responsible to maintain confidentiality, security, privacy, and integrity of protected health information.

Programmatic Duties:

Use available databases (CARE Ware, STARS, HMS, Provide, the Linkage module etc.) to identify HIV positive individuals who are not in medical care, not taking anti-retroviral medication (ARVs), or have tested positive but have not been engaged into care.

Contact individuals who are not in medical care, taking ARVs, or who have been tested positive and not aware of treatment options and community resources. The goal is to get the clients into medical care, initiate ARVs, and work collaboratively with DIS to ensure notification of positive status and perform partner elicitation.

Work with the local county STD Department to ensure that HIV and STD positive inmates are linked to care upon release from the county jail. Work closely with Area 8 surveillance officers to gain information on newly HIV diagnosed individuals, Area 8's medical and non-medical case management staff to identify and notify clients who are out of care from their case management database. This will also involve working with the Area Planning Groups such as the Regional HIV/AIDS Consortia (RHAC), the Prevention Planning Committee, the Continuum of Care Committee, and the Early Intervention Consultant (EIC) on patient care matters.

Miscellaneous Activities:

  • Attends and participates in general staff meetings and staff development, such as team/staff meetings and in-service/ continuing education.
    • Performs related duties as necessary including public health services during a disaster.
      • Maintains timely submission of EARs and timesheets.
        • Alternative Key Custodian: This position has the responsibility of being the Alternate Key Custodian for the HIV/AIDS Surveillance secure area(s) at the Michigan location, and, in the absence of the Key Custodian, will perform related duties as defined in DOHP 50-10. Performs other duties as assigned.

            • Certified as an HIV/AIDS Counselor and Tester, State of Florida Disease Intervention Specialist. Other job-related requirements for this position:
              • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to threats involving any disaster or threat of disaster, man-made or natural.
              • Incumbent must possess a valid Florida Driver License and have a motor vehicle to carry out these duties.
              • Must be willing to undergo FIT testing.
              • Selected immunizations or titers and/or periodic screening for tuberculosis may be recommended for this position.
              • This position is authorized to transport confidential information outside of established DOH-Lee facilities and must comply with parameters defined in DOHP 50-10.
              • This position is classified as sensitive. This individual will perform tasks defined as sensitive and handles confidential information.
              • Background screening is a condition of employment.
              • This position will abide by all state and federal laws, rules, and DOH policies and procedures. Working hours: (A) Daily from 8:00 AM to 5:00 PM (B) Total hours in workweek 40+40 (C) Explain any variation in work (split shift, rotation, etc.) Occasional weekend and/or evening hours, as required by the program needs.
